Email Newsletters/Blasts: Anyone using them effectively?

Email is another one of those topics I've done a ton of research on -- and learned a lot of things I'm personally doing WRONG. Anyone using email (i.e., for newsletters/email blasts, not just correspondence) with good returns? What program do you use (e.g., Constant Contact, Mailchimp, etc.)? How often do you send things out? What kind of content do you include? What kind of open rate do you get? Click rate? Actual sales?
Again, this is also research for an upcoming article so I may want to contact responders for the article.

I've been using MailChimp to no avail. Every spring I send out an email blast with deep discounts to prospective and current customers. If I'm lucky, out of 125 email addresses, I'll get two responses. The dead line for this campaign isn't here yet and winter liners on so we'll see if there is a difference this year.
